a computer-based surgical planning system determines locations of surgical instruments or implants relative to a coordinate system determined for a 2d or 3d model of at least a portion of a patient's anatomy. the planning system generates data, such as files or objects, from which holograms of the surgical instruments may be generated by a mixed reality (mr) or augmented reality (ar) device. the mr/ar device registers the patient's anatomy and anchors the holograms to the patient's anatomy relative to the coordinate system determined for the 2d or 3d model. the holograms include holograms of trajectories of screws for securing an implant to the patient's anatomy.
